Student/Athletes need a sports qualifying physical every 3 years. If they get their first one in 7th grade, they need another one in 10th and so on. Forms are also available at the Ely Clinic as well as from Mr. Coombe, the Ely High School Athletic Director. 6th graders do not have to have a physical on file until 7th grade (they are not participating officially yet).

MSHSL Athletic Eligibilty Forms must be completed by skiers in grades 7-12 and signed by both athlete and parent once per year. If your skier participated in a fall sport, he/she has already completed this form. 6th graders do not have to complete this form.
